Application (How to Apply)  
 

1. Conduct an internal assessment (Self-Assessment Forms: Business / Health Care) based on the relevant PQA criteria;

2. Present the results of the assessment to the Award Administrator;

3. Attend an Application Development Seminar;

4. Show proof of compliance with relevant statutory and regulatory requirements from BIR, Bureau of Customs, GSIS/SSS, DENR, DOLE, SEC;

5. Submit two (2) copies of the completed Eligibility Determination Form to the Award Administrators; and

6. If eligible, submit 10 copies of the accomplished Application Form together with other requirements (e.g. Application Report and Fees)


The Application Report shall contain the following:
